Dear Sir or Madam;
The insured party (…) (the other party responsible for the damage) is registered with your company under policy number (…), agency number (…), and the policy is a Motor Vehicle Compulsory Liability Insurance Policy.

On (…) (date), the vehicle with license plate (…)—which was under the control and management of (…) and registered in their name—collided with the motorcycle with license plate (…) operated by our client (…) in (location), resulting in a traffic accident that caused injuries. As a result of the accident, the client’s (…) right arm had to be amputated at the elbow.
The driver (…) is 100% at fault for the accident. The cause of the incident was (…) (errors and omissions in the incident will be listed). The client was driving in his own lane in accordance with the rules when he was involved in the accident. In addition, an investigation into the accident is being conducted by the TC (…) Chief Prosecutor’s Office and the (…) Prosecutor’s Office.
The client sustained serious and severe injuries as a result of the accident. On the day of the accident, the client was taken to (…) Hospital; he received intensive treatment there, and following the treatment and examinations conducted at (…) Research and Application Hospital, the client’s right arm was amputated from the elbow down.
The client continues to live with the adverse effects of the accident. Furthermore, the client sustained lacerations and fractures; the lacerations have left permanent scars on his body. Due to the grief of losing his arm and his inability to walk without assistance, he does not wish to go outside. Furthermore, he has reached a point where he will be unable to continue his education or professional life in the future. The insured party(ies) responsible for causing this suffering must compensate the client for his emotional distress.

For the reasons explained above, while reserving our right to claim and sue; and taking into account the policy mentioned above, we request that the material damages resulting from the accident (loss of earning capacity, medical expenses, loss of income, disability) and the moral damages resulting from the negative psychological and environmental factors caused by the accident be calculated and communicated to us, and that the funds be deposited into the account provided below.
