1- Since my client wishes to undergo gender reassignment, prior authorization was obtained through the Erzincan Criminal Court of First Instance’s Case Amendment Order dated …/…/… and numbered 2007/…, and Decision Amendment Order numbered 2007/….
2- After obtaining authorization from the court, my client underwent surgery. Thus, their gender has been physically changed. The Medical Board Report is attached. It has become necessary to file this lawsuit to have the gender reassignment recorded in the civil registry.
