Subject: To present my excuse for testifying as a witness.
I have been summoned as a witness by your court for the case whose main number I have indicated above. Due to health problems, I will undergo surgery at. Okmeydanı State Hospital on 25/08/2015, i.e. the date of the hearing. Therefore, it is not possible for me to be present at the court on the day and time of the hearing I have been summoned as a witness. I request that my excuse be accepted and that I receive my witness statement on the next hearing date.
